Dynamic Questionnaire?

I'm trying to understand the Dynamic Questionnaire Application. How would I go about using it?

The Dynamic Questionnaire Application can be installed to your Decisions server in the Apps store (My Apps Folder > "Not Installed" > Dynamic Questionnaire > Install). 

With the Dynamic Questionnaire Application, you can manage and send out email surveys, track responses, and design templates for future responses.
