Database already exists error during install

Hello All,
For a new instance, I am having trouble installing Decisions. Every time I reach the last step, the installer fails and I receive this as a popup.

Hello Sam,
The most common reason we received this error message is due to having a database with the name currently existing. Can we check if a database with the name specified in the error already exists? Using a different name would allow us to proceed with the install.Another common reason is that we did not change/specify what type of database we will be using. Since we are using an Azure SQL Database, lets double check that we specified the type during the database install.
