    The Max Number of Attempts is a customizable setting in Decisions. You can find this at Portal Settings → Enable Account Lockout.

    If you go to System → Security → Accounts, you should be able to reset the attempt count for an Account by right-clicking on it and then selecting the Reset Number of Attempts action.
