I have configured an SMTP server for our Decisions server, but emails are failing both from Send Email steps and for Assignment Notification emails. What should I be checking here?

The first thing to confirm is that you are not bypassing your SMTP server settings in settings.xml. If this setting is set to TRUE Decisions will not use your SMTP server settings at all and instead will attempt to send direct mail from the application server. If this is TRUE, set it to FALSE, Restart Service Host Manager(SHM) and attempt your tests again.
