Passing multiple Value Lists to a Form

Im trying to pass multiple Value lists to a form to be used to populate dropdown fields.
I have tried to create a sub flow and pass them all to a data structure so that I only have to pass the data structure to the form, but I cant get it to work.
If there is a better way to do this, I am open to suggestions.
