Create and Populate Data Repeater Row Via Active Form Flow

I need help creating an Active Form Flow that creates a new row on a data repeater when a value is chosen from a drop-down list that simultaneously populates a field on it.

I have attached a test project that can be used in Decisions Version 9.6.1.74931 or higher. The Active Form Flow contains an Add Item to List step that takes in the selected value from the drop-down list. The selected value is added to the end of the list, thus creating a new row and populating the textbox.
