Comments can be added to a Case Entitys process folder through the use of a Save Entities step with its type set to Comment[DecisionsFramework.ServiceLayer.Services.Comment]. Ive attached a demo project for cases that exhibits this in several of its flows including one named Initial A Approved/Deny.
