Finding the Default Form in Decisions

Where do i go to access the default form? Looks like someone has made a change to ours which I would like to revert. When we open the form now it doesn’t have an area at the top where a title and logo would be expected to go. We’re using 5.0.3

You should be able to find the default forms under System > Designers > Catalog > System Defaults. This is where all our Core Decisions Defaults are housed for Forms, Flows, Pages, etc. Youre probably looking for "Base Form" or "Grid Form.","6/17/2019 10:14:29 AM
