New Action Button Not Displaying

We have a dashboard with an ActionsBar. We tried to add a new button to it but the new button does not display. The Action filter is calling an Action flow:

We had an issue like this back in January and it was recommended we add the action to the folder this way:

We did that this time on the folder where the dashboard resides but the button still does not show. We tried restarting the service but no change.
