Dynamic controls on the form

[color=rgb(34, 34, 34)][font=Arial, Helvetica, sans-serif][color=#000000]In my example, I used Decisions accounts Email Addresses as account name and account States as description textbox. You may change the input data according to your data structure. [/color][/font][/color]
